When '-disinf' and '-silent' have been specified, why does the program still stop and wait for a prompt to disinfect a file?

This is because the program expects the user to either specify the '-auto' command-line switch or to supply the program with input. '-silent' is meant for scripts, but there is still a way to provide the program with input when it is run in a script.
